Healthy Sheboygan County invites you to attend our Data Walk Open House on Wednesday, Oct. 7, from 2–6 PM at Jake’s (529 Ontario Ave). Come review the latest community health data, provide feedback on our local priorities, enjoy snacks, and enter for a chance to win a gift card! This is a "come and go" event with no formal presentations. Your input is essential to our next three years of community health planning.

The 2026 Community Health Needs Assessment (CHNA) involved collecting data through a community input survey, key informant interviews, focus groups, and secondary data collection. The CHNA provides Sheboygan County with information on the health of county residents. Reports including, but not limited to, information on behavioral and lifestyle habits, risk factors, and disease conditions are available below.

The 2023 Community Health Needs Assessment (CHNA) included a phone survey, community input survey, key informant interviews, listening sessions, and secondary data collection. The CHNA provides Sheboygan County with information on the health of county residents.
